Premature Mortality Burden Related to Pulmonary Embolism in Cuba

Background: Despite improvements in recent decades, mortality related to pulmonary embolism remains high and may be exhibiting an increasing trend in some low- and middle-income countries. However, little research has been done on the premature mortality burden related to pulmonary embolism in Cuba.

Aims: To identify the premature mortality burden related to pulmonary embolism in Cuba.

Methods: Data were obtained from the mortality database of the Ministry of Public Health of Cuba (International Classification of Diseases-10, codes: I26.x, I80.x, I82.2, I82.4, I82.6, I82.8, I82.9, O22.2, O22.3, O22.9, O87.0, O87.1, O87.9 and O88.2, as underlying cause of death) between 2015-2018. Years of life lost (YLL) were calculated using the standard life expectancy of the WHO’s global health estimates. Age- and sex-specific YLL rates were calculated using the Cuban mid-year population estimates and the age-standardized YLL rate (ASYR) was estimated by the direct method using the WHO’s standard population.

Results: A total of 2292 pulmonary embolism-related deaths were identified, 1131 (49.3%) male and 1161 (50.7%) female. The median age at death in men was 75 years (interquartile range[IR]): 63 to 83) and in women 77 years (IR: 66 to 86).  A total of 40 568 YLL were accumulated, with an average of 17.7 YLL per death. The absolute YLL count and crude YLL rate according to age, sex and causes of death are shown in Figure 1 and Table 1. ASYR between 2015 and 2018 increased by 90.6% in men (43 vs 82 YLL per 100 000 inhabitants) and by 52% in women (48 vs 73 YLL per 100 000 inhabitants).

Conclusions: Premature mortality burden related to pulmonary embolism in Cuba is high and appears to be increasing, affecting more males and the elderly. Further epidemiological research is needed to improve our understanding of this condition and its impactin our setting.
